Question 1 of 10 · Professional practice and ethics

A counselor receives a subpoena requesting a client's counseling records. The client has not authorized disclosure. What is the most appropriate initial action?

  1. A.Immediately provide the complete chart to the requesting party.
  2. B.Destroy the records to protect confidentiality.
  3. C.Determine the applicable legal and ethical obligations and seek appropriate consultation before releasing information.
  4. D.Tell the client that any subpoena automatically overrides confidentiality.
Reveal answer and rationale

Correct answer: C

A subpoena should prompt careful review of the legal requirements, confidentiality obligations, potential privilege, and appropriate consultation. A subpoena is not necessarily the same as a court order directing disclosure. Requirements depend on jurisdiction and circumstances.

Question 2 of 10 · Intake, assessment and diagnosis

During intake, a client says, “Sometimes I wish I could disappear and never wake up.” What should the counselor do next?

  1. A.Assume the statement is only a figure of speech.
  2. B.Explore suicidal thoughts directly and assess immediate safety, intent, plan, and protective factors as indicated.
  3. C.Change the subject so the client does not feel embarrassed.
  4. D.Wait until the third session before asking about safety.
Reveal answer and rationale

Correct answer: B

A statement suggesting possible suicidal ideation warrants a direct, nonjudgmental safety assessment. A counselor should clarify what the statement means rather than assuming there is no risk or automatically concluding that a specific level of risk exists.

Question 3 of 10 · Social and cultural diversity

A client says the counselor misunderstood the importance of their family's cultural traditions. Which response best supports culturally responsive practice?

  1. A.Tell the client that effective counseling is culturally neutral.
  2. B.Assume that every member of the client's cultural group holds the same values.
  3. C.Redirect immediately to symptom reduction.
  4. D.Invite the client to describe what the tradition means to them and consider how the counselor's assumptions may have affected the interaction.
Reveal answer and rationale

Correct answer: D

Cultural humility involves curiosity about the individual's own experience and awareness of the counselor's assumptions. A counselor should avoid stereotypes while making room for a client's understanding of their cultural context.

Question 4 of 10 · Human growth and development

In Erikson's psychosocial theory, the developmental task commonly associated with adolescence centers on which conflict?

  1. A.Identity versus role confusion
  2. B.Integrity versus despair
  3. C.Generativity versus stagnation
  4. D.Trust versus mistrust
Reveal answer and rationale

Correct answer: A

Erikson associated adolescence with identity versus role confusion: exploring values, commitments, and a sense of self. The other conflicts correspond to different stages of psychosocial development.

Question 5 of 10 · Assessment and testing

A newly developed inventory gives nearly identical scores when people repeat it, but its items fail to measure the construct it claims to assess. Which interpretation is most accurate?

  1. A.It must be valid because its scores are consistent.
  2. B.It is both reliable and valid.
  3. C.It may show reliability without sufficient evidence of validity.
  4. D.Its scoring method makes validity irrelevant.
Reveal answer and rationale

Correct answer: C

Reliability concerns consistency of measurement; validity concerns whether evidence supports the intended interpretation and use of scores. Consistent measurement alone does not establish that a tool measures the intended construct.

Question 6 of 10 · Counseling skills and interventions

A client says, “I got one question wrong, so I always fail when things matter.” Which CBT strategy most directly examines the client's inference?

  1. A.Ask for evidence supporting and contradicting the idea that one mistake means always failing.
  2. B.Agree that the client is unlikely to improve.
  3. C.Change the topic to avoid increasing anxiety.
  4. D.Focus exclusively on breathing without discussing the thought.
Reveal answer and rationale

Correct answer: A

Collaborative cognitive restructuring examines whether the conclusion is supported by the evidence. The statement generalizes from a single event to a sweeping negative prediction.

Question 7 of 10 · Group counseling

A counselor is orienting participants to a new counseling group. Which statement about confidentiality is most appropriate?

  1. A.The counselor can legally guarantee that no participant will ever repeat anything.
  2. B.Confidentiality expectations, the counselor's obligations, and the limits of control over other members' disclosures should be discussed before participation.
  3. C.Confidentiality is irrelevant because the group includes several clients.
  4. D.Only the group leader needs to understand confidentiality rules.
Reveal answer and rationale

Correct answer: B

Group informed consent includes discussing confidentiality and its practical limitations. Counselors can establish expectations and describe their obligations, but cannot guarantee that every other participant will maintain confidentiality.

Question 8 of 10 · Treatment planning

Which treatment objective is the most clearly measurable and behaviorally specific?

  1. A.The client will feel better soon.
  2. B.The client will stop worrying forever.
  3. C.The client will become a completely different person.
  4. D.The client will practice a five-minute grounding exercise twice per week for four weeks and review its usefulness in session.
Reveal answer and rationale

Correct answer: D

A useful measurable objective specifies an observable action, frequency, and time frame, which can be reviewed collaboratively. Treatment goals should also be individualized and meaningful to the client.

Question 9 of 10 · Core counseling attributes

A client says, “I talk and talk at home, but nobody really hears me.” Which response best illustrates reflection of feeling?

  1. A.You should set more rules for your family.
  2. B.It sounds painful to feel unheard by the people closest to you.
  3. C.Perhaps your family members are simply busy.
  4. D.Can you list everything you said yesterday?
Reveal answer and rationale

Correct answer: B

Reflection of feeling communicates an empathic understanding of the emotional meaning of the client's words. Advice, reassurance, and fact-gathering may have uses, but they are not examples of reflecting feeling.

Question 10 of 10 · Research and program evaluation

A counseling program administers a wellbeing survey before and after a new workshop. Scores improve, but there is no comparison group. What conclusion is most defensible?

  1. A.The workshop definitely caused the entire improvement.
  2. B.The results prove that the workshop will help every future participant.
  3. C.The improvement is promising, but other explanations cannot be ruled out without a stronger design.
  4. D.The survey is unnecessary because all program evaluations are subjective.
Reveal answer and rationale

Correct answer: C

A pre/post change suggests an association but does not, by itself, establish causation. History, maturation, selection, and measurement effects may offer alternative explanations.

What to do after reviewing these questions

Explain the reasoning behind each correct answer in your own words. Review concepts you missed, practice them again in new contexts, and use a broader check to decide what needs more attention. An isolated practice score is not an NCE passing prediction.
